The SDoH Action Tool - Data Sources

The SDoH Dashboard Action Tool includes a comprehensive database containing linked health care measure data with free and publicly available SDoH datasets and is maintained using Tableau. Data from the SDoH Dashboard Action Tool are obtained from the American Community Survey (ACS), the Opportunity Atlas, Food Access Research Atlas, the National Walkability Index, and the CMS Mapping Medicare Disparities (MMD) Tool. The SDoH Dashboard Action Tool includes data that is at least 95% complete at the national level. We do not include an indicator if it is missing data for at least 5% of census tracts (for SDoH measures) or counties (for health outcomes). The SDoH Dashboard Action Tool also allows users to aggregate SDoH and health outcome data to the state and national levels for comparisons.

  • American Community Survey (ACS) is an ongoing survey that provides vital information on a yearly basis about our nation and its people. Information from the survey generates data that help determine how more than $675 billion in federal and state funds are distributed each year. 
  • The Opportunity Atlas captures social mobility data with longitudinal information at the neighborhood level. This data was measured by studying economic opportunity across geographic areas.
  • Food Access Research Atlas contains food access indicators using different measures of supermarket accessibility.
  • National Walkability Index dataset includes nationwide geographic data that ranks communities on their relative walkability.
  • CMS Mapping Medicare Disparities (MMD) Tool contains health outcome measures for disease prevalence, costs, hospitalization for 60 specific chronic conditions, emergency department (ED) utilization, readmissions rates, mortality, preventable hospitalizations, and preventive services.
