The Center for Medicare & Medicaid Innovation has implemented a new service delivery and payment model for Medicare beneficiaries with End-Stage Renal Disease (ESRD). The goal was to improve health outcomes, quality of care, and efficiency for this vulnerable, expensive population. The design followed that of an accountable care organization for ESRD beneficiaries. It provided financial incentives to dialysis facilities, nephrologists, and other Medicare ESRD providers and suppliers to collaborate and address the needs of these beneficiaries while improving outcomes and reducing costs.
IMPAQ leveraged its Health and Technology Solutions Divisions to conduct data analyses using the Centers for Medicare & Medicaid Services’ Integrated Data Repository and provide raw claims data to program participants. We also integrated data from multiple sources to implement the Comprehensive ESRD Care Model’s quality strategy.
